Your health and lifestyle

Thanks for giving us a little information about who you are.

We have now reached the health and lifestyle section of the registration process. We need to take a baseline assessment from you to help us understand how we can help.

We're going to ask a few questions about your current overall physical activity levels, height and weight, and if you smoke or drink alcohol. Don't worry no one is going to be judging you, so try to be as honest as you can.

Get active

One of the best things we can do to stay healthy is to be active.

In the past week how many days have you been physically active for a total of 30 minutes of more?

Lose weight

Maintaining a healthy weight is important for health. In addition to lowering the risk of heart disease, stroke, diabetes, and high blood pressure, it can also lower the risk of many different cancers.

The easiest way to work out how your weight stacks up is by calculating your Body Mass Index (BMI). Just let us know your height and weight and we'll do the rest.

Stop smoking

Quitting smoking is one of the hardest things to do but it is also one of the most important things you can to for your health.

Drink less

Many alcohol-related health risks don't appear until later in life.

Cutting down on how much alcohol you drink will reduce your risk of developing serious health issues such as cancer, liver or heart disease and could contribute to lowering your blood pressure.

How does this work?

The tailored approach LiveWell Dorset uses to come up with solutions to the things that are stopping us achieving our goals, is based upon work done by University College London developing the COM-B model of behaviour change. In this model, all behaviour is influenced by understanding a person’s capability to change, their opportunity to change and their motivation to change. By understanding which of these is the biggest barrier to change, we can tailor support accordingly. This model of behaviour change is at the centre of the support we offer, online and in person.
