What you need to know about stopping weight loss drugs

There are a number of weight loss medications like Tirzepatide (known more commonly as Mounjaro® or Zepbound®) that have helped people lose weight and improve their health. These are injectable treatments which work to copy our natural hormones, by regulating our hunger levels and blood sugar. They slow digestion, reduce cravings and help you to feel full sooner.  

But at some point, you or your prescriber might decide it’s time to stop. Whether you’ve reached your goals or are experiencing side effects, here’s what to expect and how to prepare. 

Why might someone stop taking weight loss drugs? 

There are lots of different reasons but here are just a few:  

  • You’ve reached your weight or health goals. 
  • You’re experiencing side effects. 
  • Cost or access is an issue. 
  • You’re planning a pregnancy. 
  • Your prescriber recommends it for medical reasons. 

Whatever the reason, stopping should always be done with your prescriber’s guidance. Never stop or change your dose on your own. 

What happens when you stop?

Your body will notice the change but don't be alarmed. Here’s what to expect: 

Potential for weight regain 

Without the medication’s support, it’s easier to regain weight. This is a normal response from our bodies, not a personal failure.

Research shows that it is very common for people to regain some of the weight they lost, so it is very important to remember to keep going with those healthy lifestyle changes as you begin to stop the injections.  

Return of hunger and “food noise” 

Weight loss drugs help to make you feel less hungry. They also help to stop you thinking about food quite as much. When you stop, these things might return quickly. You may: 

  • Feel hungrier more often and experience stronger cravings 
  • Think about food more frequently 

Being aware of this helps you prepare mentally and emotionally. 

Changes in blood sugar levels 

Tirzepatide can also be used for diabetes. Even if you take it just for weight loss, stopping it can still change your blood sugar.

Your prescriber may want to keep an eye on this, especially if you have insulin resistance or prediabetes. 

How to stop safely...

Slowly vs. suddenly  

Your prescriber may recommend tapering. This means gradually lowering your dose over many weeks. This is helpful because it lets your body adjust and reduces the risk of cravings or weight gain. 

Example (your prescriber will talk you through this): 

  • Week 1-2: Slightly lower dose 
  • Week 3-4: Lower again 
  • Week 5 onwards: Stop completely 

Keep a diary of how you are feeling. Include things like hunger, mood, sleep and weight, this way you can keep track and share it with your prescriber during check-ins. 

Consider a maintenance dose 

Some people benefit from continuing Tirzepatide at a maintenance dose. A maintenance dose involves making sure you always keep the right level of the drug in your body, making sure the drug keeps working steadily over time, rather than giving you a quick boost. 

  • It may be the same or slightly lower than your weight loss dose. 
  • The goal is to prevent weight regain and maintain health benefits. 
  • Your prescriber will help decide if this is right for you. 

Maintaining weight loss after stopping 

This is the most important part of the journey. Here’s how to stay on track: 

Nutrition 

  • Get lots of lean proteins (turkey, fish, chicken), vegetables (broccoli, peppers, carrots) and whole grains (oats, quinoa, brown rice).

  • Avoid processed foods and sugary snacks. 

  • Plan meals and snacks to avoid snacking.

  • Stick to regular meal times to steady hunger levels.

Physical activity 

  • Aim for 150+ minutes of moderate activity weekly (raised heart rate). 

  • Include strength training to keep muscles strong.

  • Choose activities you enjoy to stay committed. 

Mindset and habits 

  • Practice mindful eating: this means slowing down and listening to your body. 

  • Manage stress with tools like breathing exercises, journaling, or hobbies. 

  • Stay connected with supportive people – friends, family or support groups.

Monitoring 

  • Weigh yourself weekly to spot trends early. 

  • Use a food diary or app to track meals and habits. 

  • Continue seeing your prescriber for ongoing support. 

 

 

What not to do

  • Don’t stop suddenly without advice from your prescriber. 

  • Don’t change your dose based on online advice. 

  • Don’t share or use someone else’s medication. 

  • Don’t expect to feel exactly the same, your body needs time to adjust.
