Consent to Share

Public Health Dorset supports Dorset County Council, Bournemouth Borough Council and Poole Borough Council to improve the health and well-being of local people by providing a health improvement service called LiveWell Dorset (LWD). We deliver brief advice as well as 1:1 support through our Wellness Coaches.

When you registered with LiveWell Dorset, we asked you some questions about you and your health (how much you smoke, how active you are, how much you drink and how much you weigh) We’ll store that information and use it to shape our service offer to you – such as the advice we give or the extra services we connect you to. The only people who will see this information will be those involved in the delivery and management of the LiveWell Dorset service. If you would like to use extra services (such as slimming clubs and pharmacies) we have to share this information with them. For us to be able to put you in touch with these services, we must have your consent to share that information with the service. Information will not usually be shared without your consent. But there are some circumstances where we are allowed by law to do this. Some examples of this are safeguarding or criminal issues.

We’ll be in touch 3, 6 and 12 months from today to see how you’re getting on. To do that we’ll ask you the same questions we’ve asked you today. We will hold your personal information for 5 years after your last follow up at 12 months, then it will be deleted.

Personal information we hold about you will be held safely and securely. All telephone conversations are recorded for quality and training purposes, and data stored for whilst you are registered as an active client.

You have the right to ask to see your personal information we hold about you and you have the right to ask for your personal data to be deleted, at any time, by informing the LiveWell Dorset service.

If you think there is a problem with the way we are handling your data. You can complain to the Information Commissioners Office. If you want to know more information, please speak to the LiveWell Dorset staff.

As indicated by the checklist below, you have consented to the following:

  • For your personal information being shared with the organisations listed, and at times, for our contact to be supported by technology partners, who have the same data protection standards and safeguards as we do
  • For the organisations listed below to contact you in relation to your care.

How does this work?

The tailored approach LiveWell Dorset uses to come up with solutions to the things that are stopping us achieving our goals, is based upon work done by University College London developing the COM-B model of behaviour change. In this model, all behaviour is influenced by understanding a person’s capability to change, their opportunity to change and their motivation to change. By understanding which of these is the biggest barrier to change, we can tailor support accordingly. This model of behaviour change is at the centre of the support we offer, online and in person.
