Do you agree to share your information with the LiveWell Dorset team (part of Public Health Dorset)?

When you register for the LiveWell Forum, we will store your details so that we can invite you to take part in Forum activities/events. We'll ask some general details about yourself and whether you have used the LiveWell Dorset service before. These details will help us decide who to invite to different activities, as sometimes we will be looking to talk to certain people e.g. male/female, or from a particular town/area. If you are or have been a LiveWell Dorset client, we may refer to your client record to help with this. If relevant, we will use any information you give about special needs to make sure we allow for these.

The details you share here will only be used for the purpose of contacting you about Forum activities. They will only be accessed by a small number of people on the Public Health Dorset team who are involved in organising the Forum. When you get involved in a Forum project, we may want to gather more information at that point. If we do, we will let you know how we’ll handle that information at the time. You always have the right to say no to sharing further information about yourself. 

Your details will be stored securely and will only be kept for as long as they are needed. e will delete your details from the Forum database after five years. You will be able to opt-out of the Forum at any point. If you do so we will delete your details as soon as possible and within at least 6 months, allowing for information from any active involvement you may have had to be analysed and anonymised before we do so. You have the right to ask us to delete your information at any time.  You are able to opt-out of the Forum at any point. 

If you are or have been a LiveWell Dorset client, our Privacy Notice still applies. The above only relates to the information you share when you register to join the Forum.

Before we sign you up for the Forum, please confirm that:

  • You're happy for your personal information to be stored by Public Health Dorset
  • You understand how we will use the information you share in this survey
  • You understand that you can withdraw your consent at any time and that we will only keep your data for as long as we need to

How does this work?

The tailored approach LiveWell Dorset uses to come up with solutions to the things that are stopping us achieving our goals, is based upon work done by University College London developing the COM-B model of behaviour change. In this model, all behaviour is influenced by understanding a person’s capability to change, their opportunity to change and their motivation to change. By understanding which of these is the biggest barrier to change, we can tailor support accordingly. This model of behaviour change is at the centre of the support we offer, online and in person.

Do you provide your consent to share your information with the LiveWell Dorset team – part of Public Health Dorset ?

When you register with LiveWell Dorset, we ask you some questions about you and your health (how much you smoke, how active you are, how much you drink and how much you weigh). We store that information and use it to shape our service offer to you – such as the advice we give or the extra services we connect you to. The only people who will see this information will be those involved in the delivery of the service and management of the data. If you would like to use extra services (such as slimming clubs and pharmacies) we have to share this information with them. For us to be able to put you in touch with these services, we must have your consent to share that information with the service (we’ll ask for that later, when if you sign up for extra services).

Before we can sign you up any further, we need to know:

  • You're happy for your personal information to be shared with LiveWell Dorset.
  • You understand what information may be shared and why, and that at times, our contact to you may be supported by technology partners, who have the same data protection standards and safeguards as we do
  • You acknowledge that you can withdraw your consent at any time by informing LiveWell Dorset.
  • You understand that if you do not give consent or withdraw consent then it could be difficult for us to connect you some of the services we offer.
